It doesn't mean the ingredient is safe. It means we don't have the full picture, and that uncertainty is the very reason caution makes sense.

There's also a route of exposure that goes beyond systemic absorption. Anything applied to the chest, nipple, or areola can be directly ingested by the baby during feeding. That's a completely separate mechanism from bloodstream transfer, and it matters for how you think about where you're applying certain products. Molecules that bind to plasma proteins are less likely to diffuse into milk. Small molecules cross more easily than large ones. These factors explain why some ingredients on this list are higher-concern than others, even when their systemic absorption rates are similar.

The practical implication: water-soluble actives like vitamin C and niacinamide are lower-risk transfers even when they do enter the bloodstream. When you can't prove something is safe for an infant and you have alternatives that work, you use the alternatives. This isn't paranoia. It's rational risk management in a context where the stakes are asymmetric. The downside of unnecessary caution is that you use a slightly less aggressive active. The downside of ignoring a real risk is more consequential.

That framing is important because not every ingredient on the "to avoid" list carries the same level of documented risk. Some, like oral retinoids, are near-absolute. Others, like low-concentration salicylic acid on the face, sit in a zone where the guidance is precautionary rather than certain. Retinoids: Retinol, Tretinoin, Retinoic Acid, and All Their Relatives

Retinoids are the gold standard of anti-aging skincare. Decades of research back them. They're one of the most studied topical actives in all of dermatology. They increase cell turnover, stimulate collagen, fade pigmentation, smooth texture, and reduce the appearance of fine lines. If you've been using a retinoid, you know what it does for your skin.

And while you're breastfeeding, they're consistently flagged as ingredients to put on pause.

Here's the science. Retinoids are vitamin A derivatives. Vitamin A is a fat-soluble nutrient, which means it behaves differently in the body than water-soluble compounds. It stores in fat tissue and the liver rather than being rapidly excreted. Vitamin A toxicity is well-documented. Hypervitaminosis A causes a specific constellation of symptoms and, critically, vitamin A derivatives are established teratogens at high doses. Oral isotretinoin (Accutane) carries a black-box warning for this reason, with extremely strict prescribing protocols that require pregnancy testing because the risk of severe birth defects is so high.

The actual systemic absorption of topical retinoids is low compared to oral forms. That's a real distinction. When you apply retinol to your face, your body doesn't absorb the full amount into your bloodstream. Studies on topical retinoid absorption suggest that systemic levels from facial application are orders of magnitude lower than therapeutic oral doses. Most of the molecule stays local.

But "low" is not "zero," and the specific data on retinoid levels in breast milk from topical use is essentially nonexistent because the studies have not been done. The medical community has never set up a controlled trial to measure how much topical retinol ends up in breast milk, because doing so would require deliberately exposing nursing infants to a potential risk. So the guidance defaults to caution: the risk profile of the class, the established fat-solubility, and the absence of safety data all point the same direction.

This applies across the entire retinoid family.

Over-the-counter retinols are lower-potency vitamin A derivatives. They convert through multiple steps to retinoic acid in the skin, which is why they're gentler than prescription options. But they're still retinoids. Same pause applies.

Retinyl palmitate and retinyl acetate are the weakest members of the retinoid family, found in many everyday moisturizers, eye creams, and body lotions, often at low percentages. They're furthest down the conversion chain and therefore least potent, but they're still vitamin A derivatives. The precautionary guidance covers them.

Prescription tretinoin (Retin-A, Renova) is the highest-potency topical retinoid. More effective than retinol, more absorption, more established activity. More reason to avoid.

Prescription adapalene occupies a slightly more contested space. Some sources describe adapalene 0.1% (the OTC strength) as the lowest-risk prescription retinoid because of its selective receptor binding. A few dermatologists take a more permissive view of it during breastfeeding than they do of tretinoin. But the consensus still leans toward avoiding all topical retinoids during nursing, and this is the safer position to take until more data exists.

Oral retinoids like isotretinoin are absolutely not compatible with breastfeeding under any circumstances. The systemic concentrations are orders of magnitude higher than topical applications, and the teratogenic risk is established and serious.

The good news: this is temporary. When you're done nursing, your retinoid routine comes back. You haven't lost it. You've just moved it to a later slot in your schedule.


Hydroquinone: The High-Absorbing Skin Lightener

Hydroquinone is used to treat dark spots, melasma, and post-inflammatory hyperpigmentation. It works by inhibiting tyrosinase, the enzyme involved in melanin production. It's effective. Dermatologists prescribe it. It's been used for decades.

It's also one of the highest-concern ingredients on this list during breastfeeding, for a reason that surprises many people: its systemic absorption rate through the skin is estimated at between 35% and 45%.

That number stands out. Most skincare actives are absorbed at a fraction of that rate. Hyaluronic acid barely penetrates. Niacinamide absorbs at low levels. Even retinol, which we flag for other reasons, doesn't approach those absorption numbers in topical form. Hydroquinone crosses the skin barrier in substantial amounts, enters the bloodstream, and therefore becomes a genuine candidate for appearing in breast milk in meaningful concentrations.

There are also broader safety concerns about hydroquinone beyond the breastfeeding question. Several countries, including those in the European Union, have banned it from over-the-counter cosmetics. The FDA proposed a rule years ago to reclassify it as not generally recognized as safe and effective without professional supervision. Long-term use has been associated with a condition called ochronosis, a paradoxical darkening of treated skin. And its potential carcinogenicity, while debated, remains a live concern in the literature.

None of that context makes hydroquinone appropriate for regular use when you're nursing a baby. Fortunately, the alternatives for treating hyperpigmentation while breastfeeding are genuinely good. Azelaic acid, vitamin C, niacinamide, and kojic acid all address pigmentation through different mechanisms and are considered safe during nursing. You don't have to choose between treating discoloration and protecting your infant. You just have to choose a different tool.


High-Concentration Salicylic Acid

Salicylic acid is a beta hydroxy acid (BHA) used to treat acne, exfoliate clogged pores, and smooth rough skin texture. It's oil-soluble, which allows it to penetrate into pores in a way that water-soluble acids can't. It's one of the most effective OTC acne treatments available.

In the concentrations you find in face washes and everyday toners (usually 0.5% to 2%), salicylic acid is generally considered acceptable during breastfeeding. The absorption from small facial application areas at these concentrations keeps systemic exposure minimal. The American College of Obstetricians and Gynecologists has specifically noted that low-dose topical salicylic acid is acceptable during both pregnancy and nursing.

The concern shifts when you move into higher concentrations or larger application areas.

Professional chemical peels can contain 20% to 30% salicylic acid concentrations. At those levels, the ingredient is applied over broader surface areas, often left on for extended contact time, and penetrates significantly more deeply. The systemic absorption profile at these concentrations is meaningfully different from a 2% face wash. These are a clear no during breastfeeding.

Body treatments and washes marketed for body acne or keratosis pilaris sometimes contain salicylic acid. The concentration may look similar to a facial product, but the application area covers orders of magnitude more skin. More surface area means more total absorption even at the same percentage concentration. Spreading a 2% salicylic acid body wash across your back, chest, and arms every day is a different exposure than using a 2% facial toner on your face.

Application near the breast or nipple area is a separate concern regardless of concentration. Any topical ingredient in this zone can be ingested directly by the baby. This is an additional reason to keep any salicylic acid products away from the chest entirely during nursing.

If you use a low-concentration salicylic acid product on your face for acne and nothing more, most guidance treats this as low-risk. If you're using high-concentration peels or applying salicylic acid products broadly across your body, those are the specific applications to stop.


Chemical Sunscreen Filters: Oxybenzone, Octinoxate, Homosalate, and Their Relatives

Sun protection during breastfeeding is not optional. UV exposure drives hyperpigmentation, accelerates skin aging, and raises skin cancer risk. If you're going to be outdoors, you need SPF. The question is which type.

Chemical sunscreen filters work by absorbing UV radiation and converting it to heat. They accomplish this by being absorbed into the skin, which means they do enter the bloodstream. This is by design. The ingredient needs to penetrate to do its job. But that absorption mechanism is exactly what creates the concern during breastfeeding.

Oxybenzone (benzophenone-3) is the most studied and most concerning. The FDA, in a pivotal 2019 study, found that oxybenzone reached blood plasma concentrations that exceeded the safety threshold the agency had set, after just a single day of use covering 75% of the body. It has also been detected in breast milk. Beyond absorption, oxybenzone has demonstrated endocrine-disrupting activity in animal studies, meaning it interferes with hormone signaling. This is one of the cleaner cases for switching to an alternative.

Octinoxate (ethylhexyl methoxycinnamate) has been found in human plasma, urine, and breast milk. Like oxybenzone, it has shown endocrine activity in some research contexts. It's in the same category of concern.

Homosalate was specifically called out by the FDA as requiring additional safety data because the existing evidence doesn't establish its safety at the concentrations used in commercial sunscreens. The agency found systemic absorption levels higher than what it considers safe without more study. Its presence in breast milk has also been documented.

Octisalate, octocrylene, and avobenzone round out the list of common chemical UV filters. Octisalate and octocrylene have been found in plasma and breast milk. Avobenzone has lower absorption than some others and is sometimes treated as a less urgent concern, but for simplicity and safety, it's easier to switch to mineral protection entirely rather than trying to parse individual risk levels across the chemical filter category.

The swap here is easy, and the alternatives have improved enormously. Mineral sunscreens using zinc oxide and titanium dioxide sit on top of the skin and physically reflect UV rays. They don't rely on absorption to function. Systemic uptake from zinc oxide and titanium dioxide is minimal. Both have long safety records for use in all populations including infants, pregnant women, and nursing mothers.

Mineral formulations today are a far cry from the thick white pastes of a decade ago. Micronized zinc oxide blends cleanly into skin and can be formulated into lightweight, tinted, and skin-toned-matching products that perform beautifully. Look for a broad-spectrum mineral SPF 30 or higher with zinc oxide and titanium dioxide as the only active ingredients. Formaldehyde and Formaldehyde-Releasing Preservatives

Formaldehyde itself rarely appears on a skincare label. But it appears indirectly through a family of preservatives that slowly release formaldehyde over the shelf life of the product. These formaldehyde releasers are used to prevent bacterial and fungal contamination in water-containing products.

The names to look for include DMDM hydantoin, imidazolidinyl urea, diazolidinyl urea, quaternium-15, 2-bromo-2-nitropropane-1,3-diol (also called bronopol), and sodium hydroxymethylglycinate.

Formaldehyde is a known human carcinogen at inhalation exposures. At the levels present in most cosmetic formulations, the immediate acute risk is low. But the concern during breastfeeding isn't primarily about the immediate effect on you. It's about a substance with established toxicity at higher exposures potentially reaching an infant through milk, combined with the fact that safer alternatives are widely available.

Many formulations have already moved away from formaldehyde releasers toward phenoxyethanol, ethylhexylglycerin, and other preservation systems that carry cleaner safety profiles. Finding products without formaldehyde releasers is genuinely easier now than it was five years ago. Parabens: The Preservative Hormone Mimics

Parabens are synthetic preservatives that have been used in cosmetics for decades. They're effective, inexpensive, and stable. They're also among the more extensively studied cosmetic ingredients, which is why we know as much about their behavior as we do.

Two specific properties put them on the caution list for breastfeeding.

First, parabens are endocrine disruptors. They mimic estrogen by binding to estrogen receptors in the body. The potency is much weaker than natural estrogen, and the concentrations in individual cosmetics are low. But the concern isn't any single product in isolation. It's cumulative exposure across multiple products used throughout the day. Body lotion, shampoo, conditioner, facial moisturizer, toner, mascara, and deodorant can all contain parabens, and the combined exposure is meaningfully higher than any single product would suggest.

Second, parabens have been detected in breast milk. This isn't theoretical. Studies have measured paraben concentrations in human breast milk and found them. The clinical significance of those concentrations for a nursing infant isn't fully resolved, but the transfer itself is documented.

Fragrance (parfum) in general is worth reconsidering during breastfeeding even when the specific components aren't known to be harmful. Fragrance formulas are proprietary and not disclosed on ingredient labels. A product that lists "fragrance" could contain dozens of individual compounds, including potential sensitizers, endocrine disruptors, and synthetic musks. Synthetic musks, a common fragrance family, have been detected in breast milk.

Benzoyl Peroxide: A Nuanced Middle Ground

It metabolizes on contact with skin to benzoic acid, a compound found naturally in many foods. This metabolism limits how much intact benzoyl peroxide actually reaches systemic circulation.

At standard OTC concentrations (2.5% to 5%) applied to the face for spot treatment, benzoyl peroxide is generally considered acceptable during breastfeeding. The practical caveats: don't apply it to the breast or chest area, and don't use very high concentrations (above 5% to 10%) over large areas of the body. Within those limits, it's not on the hard-stop list.

Niacinamide: The Gentle Multi-Tasker

Niacinamide is vitamin B3. It's water-soluble, well-tolerated by essentially every skin type, non-irritating, and considered completely safe during breastfeeding. If there's one ingredient to lean into during the nursing period, niacinamide might be it.

What it does: reduces sebum production and pore appearance for oily and acne-prone skin. Calms redness and inflammation. Inhibits the transfer of melanin to skin cells, addressing pigmentation over time. Strengthens the skin barrier. Improves skin texture and hydration.
